New students are required by the Massachusetts Department of Public Health to provide a current Physical examination and up to date immunizations for entry.
Students entering from countries other than the United States, Canada, Australia, New Zealand or Western Europe should also submit a TB Risk Assessment Form completed by your physician.

PHYSICAL EXAMINATION REQUIREMENTS
A Physical Examination completed within one year prior to school entry is required for any new student entering without a Massachusetts school health record (from out of state) or from within Massachusetts if a recent physical is not included in the transferring school health record.
Mansfield Public School requires Physical Examinations to be provided for entry into Preschool or Kindergarten, Grade 4, Grade 7, and Grade 10.
All Physical Examinations must be performed by a Licensed Medical Physician, or a Physician’s Assistant or Nurse Practitioner practicing under the provisions of existing state law.
Parents/guardians with insufficient or no health insurance are asked to contact the school nurse.
